Health Services Scholarship Staff Application Form

The purpose of this scholarship is to assist an SMMH staff member who wishes to pursue further education in health sciences. One scholarship of $1,000 is awarded to the successful applicant.

Requirements

  1. The applicant must be a staff member for at least one year and who wishes to futher their education.
  2. The applicant must be accepted into a recognized post-secondary school program in health sciences.
  3. Many factors may be used to determine the successful candidate, such as his/her contribution, personal qualities, financial need, academic achievement, the candidate’s commitment to SMMH, and participation in community activities. The candidate’s triumphs, goals, aspirations may also be included in the consideration.
  4. Applicants are required to submit an essay of approximately 250 words with their application explaining why they feel they are an appropriate candidate to receive this scholarship.
  5. Three letters of reference, from people other than family, are required. Include the references’ contact information in the application form as well.
  6. Letters of support (a maximum of three) are welcome.

Selection Process

Using a scoring system, the Scholarship Committee will evaluate all applications and determine the top candidates. References for the top candidates may be contacted for a more personal discussion of the candidate. Upon review of all information about the top candidates a decision will be made.

The Scholarship Committee of the SMMH Auxiliary will advise the Executive of the SMMH Auxiliary of their final selection for approval. After approval has been granted, the Scholarship Committee will forward the decision to the applicants.
