Health Services Scholarship Student Application Form

The purpose of this scholarship is to assist a deserving graduating high school student who wishes to pursue post-secondary education in the area of Health Sciences. One scholarship of $1,000 is awarded to the successful applicant.

Requirements

1. The student must be graduating from St. Dominic Catholic Secondary School, Bracebridge and Muskoka Lakes Secondary School or Gravenhurst High School.
2. The student must be in the process of applying to a recognized post-secondary school in the Health Sciences field.
3. Best candidates will have contributed to their school and community in a volunteer or participatory capacity. Volunteer time should be beyond the compulsory 40 hours required for their high school diploma.
4. The student is required to submit the application form and a working transcript signed by an authorized school representative.
5. Three letters of reference, from people other than family, are required. Include the references’ contact information in the application form as well.
6. Letters of support (a maximum of three) are welcome.
7. Applicants are required to submit an essay of approximately 250 words with their application explaining why they feel they are an appropriate candidate to receive this scholarship.

Selection Process

Using a scoring system, the Scholarship Committee will evaluate all applications and determine the top candidates. References for the top candidates may be contacted for a more personal discussion of the candidate. Upon review of all information about the top candidates a decision will be made.

The Scholarship Committee of the SMMH Auxiliary will advise the Executive of the SMMH Auxiliary of their final selection for approval. After approval has been granted, the Scholarship Committee will forward the decision to the applicants.
