We are thrilled that you are considering an opportunity to join our team for your student placement and gain valuable experience with us. This form is designed to gather essential information about your background, skills, and interests to help us identify how we can best support your learning here at Muskoka Algonquin Healthcare (MAHC).

Please ensure that all sections are completed accurately and thoroughly, as incomplete forms may delay the review process. After submission, our Corporate Education team will review your application and reach out if there are any questions or next steps.

Conditions of Application

I agree to complete mandatory training on personal protective equipment.
 
I understand that I will be required to provide up-to-date immunizations (including proof of 2 doses of Covid-19 and a flu shot), a recent vulnerable sector check and a mask fit test.
 
I further agree that if the mandatory documentation is not submitted, the placement opportunity may be declined and offered to the next qualified student request.
